Regulations and Required Forms
The following forms are required before riders can participate in lessons: Application and Release Form; Physician's Release Form; Physical Therapy Form (yours or ours) if rider is physically involved; Negative x-ray for ADC (for Down Syndrome riders only). Please call the Rivendell Office at 457-3365 for an application package if you are interested in riding. Remember, horseback riding is not for everyone. If you cannot be accepted, you will receive a written explanation.

Physicians
Release Form - This is a two-part form. Be sure to download
